Reconciliation for Premium Holiday Payments A-444951

Purpose:  This payroll report lists, by agency, the employees and current Life Insurance Premium amounts normally paid by the agency on the employees' behalf.  This is an informational report used by the Office of Group Insurance when there is a premium holiday.  For reporting purposes, the employees must be eligible for Life Insurance, be active and receive pay for the selected payroll.

Special Instructions:  Division of Statewide Payroll Accounting (DPA) will update the DEV directory for deductions 920 and 921, before job AU444951 is requested.  The COMPANY AMOUNT OR PCT,  0.01100 must be replaced with 0.00000.  The COMPANY AMOUNT OR PCT would not change back to the original amount until the number of identified payrolls is complete.
